    Hey Dr. Parker

    First, I want to thank you for all your advice and taking the time to reply to my questions. It means allot to me.

    Second, I was wondering if you could help shed some light on somethings. I have been taking Adderall Xr for a while now, after watching your videos and reading your posts I made changes to my lifestyle. I noticed improvements in my writing for University. I was able to effectively write papers, but I changed something in my diet/supplements. I can’t figure out what happened, but the last two weeks my thinking has changed When it comes to clarity, while writing term papers and writing in general.

    Any experience or thoughts regarding this area? I remember you said something about the D pathways, in a video or article, as it relates to dopamine, norepinephrine, etc.

    I know you dont know my history, but I know something is missing or imbalenced, but I dont know where to start looking.

    Any advice would be great.

      Shooting completely in the dark with your brief comment – but the first item that comes to mind, worth researching and considering w your doc: your lifestyle changes may actually decrease your need for meds, making your previous dosage strategy too much, thus shorting out brain work – ?

    Eric here again,

    You have really helped me. Your information has really changed the way the medications has worked. I’m starting to see results I cannot explain. Except to say THANK YOU.

    Theres an issue maybe you could help me fix. When I workout in the morning, then take the Adderall Xr, I can write my papers with such clairty, but after about 3-4 hours its gone. Then I dont get any clairity in the after my lunch in the afternoon. Any thoughts or suggestion

    I workout for 30-40 minutes, then right away, take 30mg of Adderall Xr, walk to school, and eat my first meal an hour after taking Adderall XR

    Thank you so much. My grades have gone up and I am so thankful.

    Hi Dr. Parker
    My 8 year old son is on Concerta (36mg). I don’t hear you talk about Concerta online or in your books! Is there a reason? We are seeing lots of side effects (NO appetite all day, super frustrated, tired). We are so desperate for help, but the side effects are terrible. Thanks so much!

    • Meg,
      Good point on Concerta – it’s a great second drug for me if AMPs don’t work, and our team uses MPH products regularly. It’s more forgiving, but a bit less predictable. Any stimulant side effects, indeed any psych med side effects, are most likely associated with immunity issues – in my experience around 80%.         Wow thanks for the help! Our psychiatrist just keeps going up when we tell her it’s not working. We did the IGG testing and it all came back perfect. Does that make you think then that he is at the top of his window (at 36mg Concerta)? Nasty side effects and only lasting 7 hours. You are so knowledgable! I can’t thank you enough!

          Yes, 7-8 hr is about the top of the window, and need to trim w small amount of Ritalin for PM – DOE there should not go longer than 4-5 hr. Did you investigate total IgG of specific foods? Without specific foods you’re still in the dark. If negative on IgG that might cover the full DOE for awhile. If he falls off tracking predictability something’s missed biomedically.     Hello, not sure where I fit into the therapeutic window. I have been prescribed adderall xr and IR, starting at 5mg titrating up to now 30mg, with an IR booster in the afternoon. This has been over the course of 4 months. I seemed to get the most benefit at 20mg but it didn’t last very long(4 hours). Initial recommendation was to do 2-20mg per day but due to cost(out of pocket) it was cheaper to get 30mg now seems like there is little to no effect. and the IR booster in the afternoon helps for about 2-3 hours. The only noticeable side effects are lack of appetite. Other than that I seem to not have any other issues.
    My prescriber indicates that we can try vyvanase and I would qualify for their assistance.

    Where to go from here?

      Without more careful questioning beyond the scope of comments on this article I can’t say with certainty, but can suggest some speculative ideas.

      2. Vyvanse is helpful with an improved DOE, but if metabolic challenges aren’t directly addressed you could be preoccupied with trying repeatedly to get these right for years.

        Hello, thank you for the reply. I have purchased your new book on audio and look forward to listening to it. One thing I am not seeing/reading in general on your site, is specificity. From reading over your prior book there was no specific answers to some of the questions you posed. Metabolic burn.. no specific test, that I could find was given to measure our individual metabolism/burn rate. The transit time of a persons digestion is an indicator is interesting. Where is the trade off? If Adderall XR is truly designed to last longer than 4 hours, does that mean that my metabolism is fast enough to burn through it in that time, but due to me having digestion issues(slow transit time) that I am not receiving as much benefit from it?

        Yes there may be a gluten intolerance or sensitivity. Possible digestion issues, but how do those get resolved? Fiber? Taking fiber and adderall only would seem to increase the burn rate? If I happen to be lactose intolerant and the medication I am given to take has lactose in it, what is the solution to gain the benefit for the medication as dosed ? I agree that there are a lot of potential variables involved, and certainly you would know greater than I.

          Correct. Almost any digestion issues can result in metabolic changes that can’t be measured at this moment – except by conscious application of the principles I’ve raised regarding 1. awareness of DOE, 2. measurement of DOE, 3. use of DOE to adjust dosages, consideration of outcomes.

Lactose may or may not contribute to problems. So often the milk issue is not lactose, milk sugar, but casein, milk protein. Just search this site on casein for more info. We do recommend meds w/o milk products, and often since the casein is so frequently missed problems continue. Lactose is more IgE related, and therefore more often measured while IgG casein is far more frequently the real culprit in my experience.
